Please bring copies of all pertinent medical records such as MRI, CT Scan, and current Lab results.

It would also be helpful if you would prepare and bring with you to the appointment your medication list. Please include dosing, frequency, and name of each medication.

Kindly give a minimum of 24 hours cancellation time so that we may offer this time period to another patient. Please be aware that co-payments, co-insurance (if applicable) and insurance deductibles still unmet, are due and payable at the time of service.
